About the role
Our Mission
We're building infrastructure for modern healthcare delivery.
Traditional healthcare is plagued with outdated, monolithic EHRs designed to
maximize billing outcomes. Patient outcomes and provider experiences have been
afterthoughts, as these systems have bolted on non-API-first solutions. None of
this is built for how clinically excellent healthcare is actually
delivered—longitudinally and collaboratively, with the patient at the center.
Healthie is the world's leading API-first, ONC-Certified EHR for healthcare
delivery outside of the hospital. We provide the powerful infrastructure every
scaling organization needs—EHR, scheduling, patient engagement, billing, and
more—all accessible via modern APIs and a white-labeled UI. Our platform makes
it simple for organizations of any size to launch, customize, and scale their
care delivery models without reinventing the wheel.
Today, over 2 billion API calls are made to Healthie every month, as thousands
of organizations—working with more than 21 million patients in total—rely on
Healthie to deliver clinically excellent healthcare in over 35 specialties, from
preventative medicine to complex chronic care management.
We believe in the power of technology to improve access to healthcare—and we're
building the rails that make this a reality. We work fast and with quality
because we provide business-critical, healthcare-critical software that
clinicians and patients need for a better healthcare system. We're
customer-obsessed, operate with lightning-fast processes and responses, make our
product roadmap public so customers can see what we're building, and remain
relentlessly focused on how care gets delivered.
Healthie is backed by leading investors, and while we've raised $42M to date,
more importantly, we operate with fiscal responsibility and have been profitable
for more than half of our time as a company.

About the Role
Healthie is hiring a Senior Product Manager to own billing, what patients and
practices actually see: invoices, statements, balances, payments, and refunds,
and the ledger truth underneath them.
Legacy EHRs were built to maximize what could be billed, and care bent around
that. We are building the opposite: billing that serves care delivery, where
every dollar can be explained to the practice that earned it and the patient
asked to pay it. It is also where our customers feel the difference between
software that records what happened and software that does the work. We are
making a deliberate, funded bet here: a practice should be able to complete the
entire financial life cycle of a visit, from confirming coverage before the
patient arrives to collecting the final dollar, without ever leaving Healthie.
That bet is large enough that we have split it across two squads, along a line
that will hold for many years. One owns the money we have to collect from
insurers at scale. The other, this role, owns the money we control and have to
explain to a human. Those turn out to be two different disciplines, and each
deserves its own team.
You own the question "does everyone know what they owe, and can they act on it?"
A patient should never receive a bill they cannot understand, and should be able
to pay it in a couple of taps. A biller should reconcile an account in minutes
and answer any question a patient asks on the phone without opening four
screens. A practice owner should always know their cash position. None of that
is reliably true today, anywhere in this market. Get it right and Healthie
becomes the system practices run their money on, and every organization building
on our API inherits that for free. It also unlocks the care models coming next,
since episodes of care and value-based arrangements need a billing experience
that can express them and nothing today does.
Delivering that vision also means building deep, trusted relationships with the
third-party partners that power critical parts of the financial ecosystem,
working with them not simply as vendors, but as strategic partners in what we
build and where we go next.
This is a multi-year charter where the decisions you make in your first two
quarters will still be load-bearing in five years.
Healthie sits at a rare intersection: a genuinely complex domain, an API-first
platform with real technical depth, and customers who feel your decisions in
their bank accounts. If you like problems where the details decide whether
anyone trusts the number in front of them, and where getting the foundation
right compounds for years, this is one of the best canvases in healthtech.
--------------------------------------------------------------------------------
What You'll Do
* Own the roadmap for your area and the outcome it is meant to produce. You
decide what gets built and in what order, you defend that in public because
we publish our roadmap, and you are accountable for whether practices
actually get paid faster at the end of it. When the company needs to know
where billing is going, you are who it asks.
* Stay close to the billers, practices, and enterprise customers who run their
revenue on Healthie. Their reality is the input to everything above, and it
does not reach you accurately secondhand.
* Turn the mess of how healthcare actually gets paid into something precise
enough to hand off, first to the engineers and coding agents who build it,
and eventually to agents that do the billing work itself. Ambiguity is what
keeps this work manual, so precision here is not craft for its own sake, it
is the path to automating it.
* Make the early decisions that compound, together with engineering. What we
capture, when in the life of a claim or a balance, and how it surfaces later
is what makes billing legible. Get it right up front and the product explains
itself rather than needing to be reconstructed. That thinking belongs in the
room with engineering from the start.
* Ship foundational work in slices customers can feel, rather than a rewrite
that pays off someday. Knowing where to cut is most of the skill.
* Decide what "correct" means for money, then make sure we can prove it.
Reconciliation, auditability, and clean rollout gates are part of your scope,
not someone else's cleanup.
* Work with engineering, design, QA, product marketing, and support as one
squad, and keep a heavy inbound load from becoming the roadmap by default.
--------------------------------------------------------------------------------
How We Work
* At Healthie, we are AI-enabled and AI-accelerated in how we build. We are
looking for team members who actively leverage AI-powered tools, embrace
automation, and help teams adopt modern, AI-assisted product and engineering
practices to increase velocity and quality.
* This is already how the team works. Our QA engineers build their own agent
workflows, our designers audit the codebase with them, our internal agent
takes Linear tickets and opens pull requests with a human reviewer on every
one, and our product managers trace how the system actually behaves, in the
code and in production data, before deciding what to build.
* The scarce skill has moved. Anyone can generate a document. What is hard is
knowing enough about this domain and this system to direct that work
usefully, and to recognise immediately when a confident answer is wrong. In
billing, a plausible error becomes a denied claim or a misbilled patient.
Being the person who genuinely understands how this works is worth more now,
not less.
* Convention is a feature. An experienced biller should be able to look at what
we built and know how to do their job, without training and without
unlearning habits that work everywhere else. Novelty has a real cost in
comprehension and adoption, so we spend it deliberately, in the few places
where it changes the outcome, and we expect you to say why you spent it
there.
* We do not expect you to arrive already working this way. Most people have
not. Bring the product instincts, the domain knowledge, and genuine
curiosity, and we will bring the tools and the training. What we do expect is
that you keep learning on your own, because this changes every few months and
the people who do well here do not wait to be shown.

Details
* This is a full-time, hybrid position at our NYC HQ (3 days in office).
* U.S. work authorization is required and Healthie does not provide
sponsorship.

About You
This is a systems role, not a healthcare role. You do not need healthcare or
revenue cycle experience; we will teach you the healthcare-specific parts. What
we cannot teach is the instinct for how money-moving systems work and fail, so
that is what we are hiring for.
* Experience. You have at least 5 years in product management, including 2 or
more years owning billing, payments, ledger, or other systems where
correctness has financial or regulatory consequences.
* Money-movement depth. You have shipped products that move or account for
money: billing and invoicing, payments, ledgers, subscriptions, marketplace
payouts, or accounting integrations. You know why a refund does not
retroactively unpay an invoice, what reconciliation means in practice, and
how to define a balance precisely enough that two people computing it always
get the same number.
* Ownership and prioritization. You have owned a roadmap, not just a backlog.
You can explain how you decided what came first, what you refused to do, and
how you held that line when someone senior pushed. You get there from
evidence, including time spent with the people who use the thing.
* Systems thinking. You instinctively look for edge cases and reason from first
principles about how systems fail. You think in objects, states, and
transitions rather than screens.
* Technical fluency. You read API documentation, reason about how systems
behave, and can hold a design tradeoff conversation with a staff engineer
without hand-waving. If you were never an engineer, you can still talk the
talk.
* AI fluency. You use modern AI tools as a matter of course and are
appropriately skeptical of them. You do not need to train models. You do need
to be able to say where an agent was wrong and how you caught it.
* Decisiveness. You act on incomplete information. You state your assumption,
choose, write down what would change your mind, and move.
* Sequencing. You have taken a foundational rewrite or migration and found the
slices that delivered value before the whole thing was done.
* Communication. You explain complex concepts to technical and non-technical
audiences alike, and you are comfortable defending a recommendation to
executives and changing it when the argument is good.
* Alignment. Strong alignment with Healthie's core values and culture: Respect,
Resilience, Reliability and no ego.
Bonus points if you have:
* Healthcare billing exposure of any kind, for example patient responsibility,
superbills, or working alongside a revenue cycle team.
* Built on or deep familiarity with modern payment infrastructure such as
Stripe, Adyen, or Plaid, including the unglamorous parts: disputes, refunds,
payouts, and reconciliation.
* Designed consumer-facing checkout, bill-pay, or statement experiences where
clarity measurably moved collection rates.
* Worked with double-entry accounting concepts or built products that had to
survive a finance team's audit.
* Built billing or payments products on an API or platform, where other
companies inherit your correctness.
